DHI Hair Transplant in Türkiye

Direct implantation with a fine pen, planned for careful control of angle and depth.

What DHI hair transplant is

Direct Hair Implantation, known as DHI, is a variation of the FUE approach. Follicular units are still harvested individually from the donor area, but they are implanted using a specialised pen-like instrument, often called a Choi pen or implanter pen, rather than being placed into channels that were opened beforehand.

The method is surgical and relies heavily on the skill and experience of the team. It is one of several technique choices, and it is not automatically better or worse than other FUE-based approaches for every person.

How the technique works

After local anaesthesia, grafts are extracted with a fine micro-punch, as in classic FUE. Each graft is then loaded into the hollow needle of the implanter pen. The needle creates the incision and releases the graft at a chosen depth and angle in a single motion.

Because incision and placement happen together, the practitioner can control direction and density continuously while working. The process is generally slower than other approaches, as each graft is loaded and implanted individually.

  • Extraction of follicular units from the donor area
  • Loading of each unit into the implanter
  • Simultaneous incision and implantation
  • Ongoing control of angle, depth and direction

Who is a suitable candidate

DHI may be discussed for people who wish to preserve existing hair while adding density, for work along the hairline, or for those who prefer not to have the recipient area shaved fully. Whether this is possible depends on the examination and on the design planned.

As with any hair transplant, adequate donor hair and a reasonably stable pattern of loss are generally needed. Very large areas of baldness may be more practical to treat with other techniques or across staged sessions.

  • Sufficient healthy donor hair
  • Realistic expectations about density and coverage
  • Good general health, assessed by the medical team
  • No active scalp infection or inflammatory scalp disease

Procedure day, recovery and results timeline

The procedure normally takes place in one session, though long sessions may include breaks. You will usually go home or to your accommodation the same day with aftercare instructions covering washing, sun protection and sleeping position.

Redness and small crusts are typically present for the first one to two weeks. Many transplanted hairs shed in the second to third month, and regrowth commonly becomes visible from around the fourth month. Density continues to improve gradually, and results are generally judged at roughly 12 to 18 months.

Risks and important considerations

Possible effects include swelling, scabbing, temporary numbness, folliculitis, shock loss of existing hair and uneven growth. Rarely, infection, scarring or a lower graft survival rate can occur. Native hair loss may continue after surgery.

Be wary of claims that DHI guarantees a higher density or survival rate. Ask which professionals extract and implant the grafts, how many are planned for a single day, and what follow-up support is available once you are back home.

Frequently asked questions

What is the difference between DHI and FUE?
Both usually involve extracting individual grafts. With DHI, grafts are implanted with a pen at the moment the incision is made, while classic FUE often opens channels first. The better choice depends on your case.
Do I need to shave my head for DHI?
Not always. Some designs can be done with limited shaving, but the donor area usually needs to be trimmed. Your specialist will explain what is realistic for you.
Is DHI suitable for large bald areas?
It can be used, but very large areas may need staged sessions or another technique. This is decided after an individual assessment.

Results and recovery times vary from person to person. Every treatment starts with a medical evaluation by a licensed specialist.
